New Patient Information
Your First Visit
When you take advantage of our new patient special, you won’t pay anything for your initial visit to The Smilist Dental Medford. Scheduling an appointment has never been easier, considering we are open seven days a week. Come in at the time that is most convenient for you, and our staff will greet you warmly and take your photo. Fill out your new patient forms before you come back to make the registration process even easier. It will just take us a few minutes to get you into the examination room, where Dr. Rosenfeld will perform a tooth-by-tooth examination to look for potential issues. We’ll take x-rays to get an even more in-depth look.
If issues are found during this initial checkup, we’ll put together a plan for restorative care as soon as possible. Then, we’ll perform a cleaning so you leave our office smiling brighter than ever before!
New Patient Forms
To make your first visit as easy as possible, we have uploaded our new patient forms onto our website. Please take a few moments to download the PDF, fill the forms out, and print them off to bring with you to your first appointment or, even better, email them back to us before you come in. That way, when you visit, we can get you through registration and into the dental chair without any extra time spent waiting.
Existing Patient Forms
At times, the The Smilist Dental Medford team will require existing patients to complete forms again in order to update their medical history and other personal information that’s likely to change over time. Please utilize the link below to log-in through the online portal. Please login with your username and password. Completing this step from home can help you save valuable time in our waiting room.
In House Dental Plan
We don’t want anything to get in the way of you and your best smile. That’s why we are happy to accept all major payment methods. And if you could benefit from a little extra help, consider joining the The Smilist Dental Medford In-House Dental Fee Plan. It’s not dental insurance, but a plan that we have created to help you save on all the treatments and services that healthy smiles require.
In exchange for a low annual fee, you will receive a list of services that you can benefit from at no charge, including a six month checkup and cleaning, an annual set of x-rays, and a cosmetic consultation. Additional required services are heavily discounted.
Our plans start as low as $17 per month for an individual plan. We accept patients in all health conditions, so you cannot be denied coverage. Contact us to discuss the in-house dental plan today!
Insurance Information
The Smilist Dental Medford accepts most PPO dental insurance plans, and we will also file all out of network claims to help ensure you can make the most out of your dental insurance plan. Financing Available
For patients who do not have dental insurance, or those who could benefit from breaking up their bills into convenient payments, we are happy to offer patient financing through Wells Fargo Financing and CareCredit. Patient financing helps you pay for your dental treatments at the pace that is most comfortable for you — because visiting the dentist shouldn’t stretch your budget to its limits.
